Who is eligible for services?

  • An infant born with a special healthcare need, serious medical illness, or one born "at risk" for a developmental delay.
  • A toddler having difficulty walking, talking, playing and/or understanding.
  • A child diagnosed with Autism.
  • A child diagnosed with a serious or life-threatening illness or who has been seriously injured in an accident before 21 years of age.

Regardless of the financial status of the family, any child with special needs is entitled to receive services from the case management unit.

All services are free and confidential.

The Case Management Unit serves as the single point of entry into the health care system for Warren County families of children with special needs and for infants and toddlers receiving early intervention services.

A Case Manager will:

  • Assess each child, (birth through age 21), to determine how needs are being met and discuss ways to fulfill unmet needs.
  • Provide information about the NJ Family Care Program.
  • Provide information about the Catastrophic Illness in Children Relief Fund Program.
  • Develop and review an Individual Service Plan.
  • Refer to appropriate federal, state, and local resources.
  • Coordinate and monitor the delivery of services.
  • Serve as an advocate for the child and family.
