Food Forms & Guidance

You may print the forms and complete them by hand; or, your information can be typed right on the form. When complete, the form can either be printed and mailed or saved to your computer and e-mailed to the Health Department.

Annual Food & Beverage License Application
Required annually for all food and/or beverage establishments.

Change of Owner Review
Required whenever an establishment changes owners.

Use this form to apply for approval to operate a mobile unit.

Farmers Market Application
Use this form to apply for approval to sell food or drink at farmers markets. The sale of fresh uncut produce is permitted without an application.

Temporary Food Establishment Guidelines
Temporary events are those that operate for 14 consecutive days or less. If you wish to operate a temporary food establishment, the guidelines can be found here.
